If, after reading the description of our services, you are interested in scheduling an appointment, please contact the office by telephone. We will provide you with information about available appointments, and give you a description of the fees involved.

After you contact us, we will e-mail you questionnaires to be completed prior to your first appointment. It is important to take the time to fill the questionnaires out thoughtfully, as the information provided helps direct how the assessment or consultation is conducted.

ALL FORMS MUST BE PRINTED OUT BY YOU, SIGNED, AND MAILED TO THE OFFICE BEFORE AN APPOINTMENT WILL BE MADE.

A CHECK FOR $100 payable to Dr. Danis MUST ACCOMPANY YOUR PAPERWORK.

THIS NON-REFUNDABLE $100 DEPOSIT WILL BE CREDITED TOWARD THE TOTAL COST OF YOUR EVALUATION/CONSULTATION.
